Medical College of Wisconsin is Hiring a Community Health Worker I Near Milwaukee, WI

Position Description:

In the role of Community Health Worker I, you will primarily be working in the field with specific ethnic communities. This individual must be bilingual, fluent speaker of a target refugee language (Rohingya/Burmese).

The CHW will be responsible for helping refugee patients and their families to navigate and access community health services and adopt healthy behaviors. The CHW supports medical providers and voluntary agencies (VOLAGs) through an integrated approach to care management and community outreach. As a priority, activity will promote, maintain, and improve the health of patients and their family and advocate for individuals and community health. Community outreach, home visits and leading community education sessions will be required.

RHP = Refugee Health Promotion

ARHP = Afghan Refugee Health Promotion

URHP = Ukrainian Refugee Health Promotion

Responsibilities:

  • Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
  • Follow-up with patients via phone calls, home visits and visits to other settings where patients can be found. Assist patients with completing applications and registration forms.
  • Provide health education and assist with health information to ensure client knows about their medical condition.
  • Provide referrals for services to community agencies as appropriate.
  • Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.
  • Help patients connect with transportation resources and give appointment reminders in special circumstances.
  • Advocate for patients access to appropriate linguistic services and help access to interpretation services
  • Exhibit excellent working relations with patients, visitors and staff.
  • Work closely with medical providers to help ensure that patients with special medical needs have comprehensive and coordinated care.
  • Identify and apply appropriate role definition and skilled boundaries
  • Complete contract data requirements as assigned
  • Provide/Organize culturally appropriate health education sessions and information
Knowledge, Skills, and Abilities
  • Experience in community outreach.
  • Working knowledge of various integrated systems of care in the community.
  • Good communication skills, such as listening well, and using language appropriately.
  • Successful experience in managing health of self/family OR experience in managing complex medical condition of self/family in the U.S. setting.
  • Demonstrates strong verbal, written and interpersonal communication skills.

Preferred Schedule: 8-5pm, M-Friday

Position Requirements:

Minimum Qualifications:Appropriate experience may be substituted for education on an equivalent basis.

Minimum education: High school or equivalent Minimum experience: No experience required Essential function: Bilingual, fluent speaker of a target refugee language (Rohingya/Burmese)
